The spine protects neural tissue while allowing support and motion.
C1, the atlas, supports the skull. C2, the axis, contains the dens. C3 through C7 have vertebral bodies, posterior elements, transverse foramina, and paired facet joints.
The thoracic vertebrae articulate with the ribs. Their longer spinous processes and rib attachments help distinguish this region.
Lumbar vertebral bodies are large because they carry more weight. The canal contains the distal cord, conus medullaris, and cauda equina.
The sacrum joins the lumbar spine to the pelvis. The sacral canal and neural foramina transmit lower nerve roots.
A typical vertebra has an anterior vertebral body and a posterior arch formed by pedicles and laminae. The arch gives rise to transverse processes, a spinous process, and articular processes that form the facet joints. Together, the body and arch surround the spinal canal.
FOLLOW THE SPINE FROM TOP TO BOTTOM
Sagittal images show alignment, levels, discs, and the long course of the canal.
Use the skull base and C2 in the cervical spine, the rib bearing vertebrae in the thoracic region, or the sacrum and whole spine localizer in the lumbar region. Transitional anatomy can change assumptions.
Follow their anterior and posterior margins, heights, marrow, and alignment through the full field of view.
Intervertebral discs lie between most mobile vertebral bodies. Each disc contains an outer annulus fibrosus and an inner nucleus pulposus. Endplates separate the disc from vertebral marrow.
Trace the spinal cord through the cervical and thoracic canal. The cord tapers into the conus medullaris, commonly near the L1 to L2 level in adults, with variation. The cauda equina continues below.
Parasagittal images show the neural foramina, pedicles, facet joints, and exiting nerve roots more clearly than the midline slice.
Numbering errors can happen when anatomy varies.
Use the complete localizer and the facility's approved counting method. Rib variants, a transitional lumbosacral vertebra, limited coverage, and prior surgery can make level assignment less obvious. When uncertain, document the method and obtain the required clinical clarification.
READ FROM FRONT TO BACK
Axial images show the canal, lateral recesses, foramina, and paired structures.
The vertebral body or intervertebral disc is anterior. Major vessels and organs vary by level and lie in front of or beside the vertebral column.
The spinal canal contains the cord at cervical and thoracic levels, then the thecal sac, conus, and cauda equina lower down. CSF surrounds the neural tissue.
The lateral recesses lead toward the neural foramina. Pedicles form important boundaries, and the exiting nerve roots travel through the foramina.
The paired facet joints sit posterolaterally. Laminae continue toward the midline spinous process.
Paraspinal muscles surround the posterior elements. The psoas muscles lie anterolateral to the lumbar spine.
Use slice position and localizers to confirm whether the axial image crosses a vertebral body, disc, pedicle, or foramen.
Cervical nerve roots C1 through C7 generally exit above the vertebra with the same number. The C8 nerve root exits between C7 and T1. Thoracic and lumbar roots generally exit below the vertebra with the same number. Follow the site's anatomical convention and verify the level carefully.
CSF is usually bright on T2 weighted images and dark on T1 weighted images. Cortical bone and ligaments are commonly low signal. Marrow, discs, fat, vessels, and neural tissue change appearance with weighting, suppression, age, flow, and pathology.

Keep every spine image tied to a level and a plane.
Begin from a reliable landmark and account for possible transitional anatomy.
Use the vertebral body in front and spinous process in back to orient the slice.
Locate the cord, conus, cauda equina, thecal sac, lateral recesses, foramina, and nerve roots.
Match each axial image to its location on the sagittal or coronal localizer.
Follow structures across adjacent slices rather than deciding from one image.
Remember: Count first. Then connect the sagittal overview to the axial detail.

Which structures form the front and back boundaries of the spinal canal?
The vertebral body or disc forms the anterior boundary. The posterior elements, including the laminae and ligamentous structures, form the back of the canal.
What neural structures are found below the conus medullaris?
The cauda equina nerve roots continue within the thecal sac below the conus. The filum terminale also extends inferiorly.
Why should a technologist match an axial slice to the sagittal localizer?
The sagittal view confirms the level and whether the axial slice crosses the disc, vertebral body, pedicles, or another landmark. This prevents a correct structure from being assigned to the wrong level.
Where does the C8 nerve root exit?
It exits between the C7 and T1 vertebrae. This is the transition between cervical roots that generally exit above the same numbered vertebra and thoracic roots that generally exit below.
Why is the sacrum not always enough to assume that the vertebra above it is L5?
Transitional lumbosacral anatomy can change the expected count. Use the complete localizer and the approved counting method instead of relying on one assumption.
