WATER IS ALWAYS MOVING
Diffusion imaging makes microscopic water motion visible.
Water molecules move randomly because of thermal energy. In some tissues they can move relatively freely. In others, cell membranes, crowded cells, viscosity, and tissue structure limit or redirect that motion.
Diffusion-weighted imaging, or DWI, adds strong diffusion-sensitizing gradients to make the MR signal respond to that motion.
TAG, MOVE, AND CHECK
Moving water loses more signal; restricted water often keeps more.
The sequence gives water spins a position-sensitive phase tag.
Molecules change position by different amounts during the diffusion time.
A matching gradient tests whether the spins stayed in the same place.
More motion causes more phase dispersion and signal loss. Less motion often preserves more high-b DWI signal.
This is a simplified picture. Clinical tissue contains complex barriers, compartments, blood flow, and direction-dependent motion, so the measured value is called the apparent diffusion coefficient.
B-VALUE, DWI, AND ADC
First choose diffusion weighting, then calculate the amount of diffusion.
Describes the strength and timing of diffusion sensitivity. Higher b-values create stronger diffusion weighting but usually reduce signal-to-noise.
Combines diffusion effects with underlying T2-weighted signal. Restricted tissue often appears bright at high b-value.
Uses signal change across at least two b-values to estimate apparent diffusion in each voxel. Lower ADC appears darker on standard grayscale maps.
A bright area on DWI is not automatically restricted diffusion. Long-T2 tissue can remain bright through T2 shine-through. True restriction is commonly supported when the area is bright on high-b DWI and correspondingly dark on the ADC map.
A simplified two-point calculation is ADC = −ln(Sb/S0) ÷ b. Actual ADC values depend on b-value selection, acquisition, tissue, scanner, and processing, so they should not be treated as universal diagnostic cutoffs.
THE DANCE FLOOR ANALOGY
Open floor means more movement. A crowded floor means less.
Imagine dancers moving across a room. On an open floor, they spread out and lose synchronization. In a crowded room, movement is limited and more dancers stay together. DWI detects the signal difference created by that motion.
How strongly we test for motion.
Where signal remains bright.
Whether diffusion measures low or high.
Remember: Bright DWI plus dark ADC supports restricted diffusion.
