Not every symptom is an allergy, but every meaningful change deserves attention.
Acute contrast reactions are commonly described as allergic like or physiologic. Allergic like reactions can resemble an allergy even when the exact immune mechanism is uncertain. Physiologic reactions reflect the body's response to the injection or situation and can include nausea, warmth, dizziness, anxiety, or a vasovagal episode.
The first job is not to name the mechanism perfectly. The first job is to recognize the patient's current condition, watch for progression, and activate the approved response when needed. Breathing difficulty, altered consciousness, low oxygen, significant blood pressure change, or rapid progression should never be minimized.
MILD, MODERATE, OR SEVERE
Severity depends on the complete clinical picture and whether symptoms are progressing.
Examples include limited hives or itching, a scratchy throat without progression, brief nausea, warmth, headache, dizziness, or a vasovagal response that resolves spontaneously. Observe and follow policy because the picture can change.
Examples include diffuse hives, facial swelling without breathing difficulty, throat tightness or hoarseness without breathing difficulty, persistent vomiting, or mild wheezing. These symptoms can progress.
Examples include swelling with breathing difficulty, stridor, significant hypoxia, severe bronchospasm, shock, seizure, severe arrhythmia, or cardiopulmonary arrest. Activate the emergency response immediately.
A patient may become pale, sweaty, nauseated, dizzy, hypotensive, and bradycardic. This pattern differs from a typical allergic like reaction, but it can still require treatment and close monitoring.
Pain, swelling, tightness, or burning near the IV site may mean contrast has leaked outside the vein. Stop the injection and follow the facility's assessment, care, and documentation process.
If symptoms do not fit neatly into one category, focus on airway, breathing, circulation, mental status, vital signs, and progression while qualified help is coming.
USE THE P.A.U.S.E. RESPONSE
A calm sequence helps the team move quickly.
Stop the injection when appropriate, stop the scan, speak to the patient, and do not leave them unattended.
Check airway, breathing, circulation, mental status, symptoms, IV site, and vital signs within your training and local process.
Summon the qualified response team early. Use the site's emergency pathway when symptoms are severe, progressing, or unclear.
Position, oxygen, monitoring, emergency equipment, and medications must follow current orders, standing protocols, professional scope, and trained supervision.
Document the agent, dose, timing, symptoms, vital signs, interventions, response, notifications, and disposition according to policy.
This lesson does not teach medication dosing.
Emergency drugs and doses depend on age, weight, symptoms, route, training, local law, and facility protocol. Use the current contrast reaction card, approved emergency algorithm, and qualified clinical direction available at the site.
Staff should know how to call for help, where emergency equipment is kept, who may administer medications, how to remove a patient from the scanner safely, and how to document and report the event. Equipment and medications must be checked according to facility policy.

No. The reaction may remain mild, resolve, or progress. Stay with the patient, assess according to training, notify the appropriate clinician, monitor for change, and follow the facility algorithm.
A patient reports throat tightness and their voice sounds hoarse. Why is this important?
Throat tightness or hoarseness can signal a moderate allergic like reaction and may progress toward airway compromise. Stop, assess, summon qualified help promptly, and follow the emergency protocol.
A patient becomes pale and sweaty with a slow pulse and low blood pressure. What pattern may this suggest?
A vasovagal reaction is possible. The complete clinical picture still matters. Assess, call the appropriate help, monitor closely, and treat according to the approved protocol.
The IV site becomes swollen and painful during injection, but the patient has no breathing symptoms. Is this an allergic like reaction?
This pattern suggests possible extravasation rather than an allergic like reaction. Stop the injection and follow the facility's IV site assessment, care, escalation, and documentation process.
Why should contrast reaction details be documented carefully?
Accurate documentation supports ongoing care, future contrast planning, patient communication, quality review, and a clear record of the symptoms, response, and outcome.
